General Tufts Guidelines

BEFORE the study begins:

  1. Fax the DXA Equipment Checklist (Appendix 1 of DXA SOP) PDF
  2. Fax the CT Equipment Checklist (Appendix 1 of CT SOP) PDF
  3. Send 1 CT Test scan (Section 3.0 of CT SOP)

When sending in DXA scans:

Please send the following when submitting DXA scans:

  1. 3.5" floppy disk or CD-ROM (correctly labeled with Study Name, Site Number, Patient ID Number and Date of Scan)
  2. Paper printout of the subject’s unanalyzed DXA scan reports
  3. DXA Case Report Form (CRF) (Appendix 3 of DXA SOP) PDF

Without this information, analysis will be delayed.

When sending in CT scans:

Please send the following when submitting CT scans:

  1. CD-ROM (correctly labeled with Study Name, Site Number, Patient ID Number and Date of Scan)
  2. CT Data Form (Appendix 3 of CT SOP) PDF

Without this information, analysis will be delayed.

When labeling DXA disks/CDs:

You must include the following when labeling DXA:

  1. Study Name
  2. Site Number
  3. Patient ID or Number
  4. Date of scan
    1. For DXA scans, the date on the disk, and Case Report Form must match the date shown on the machine’s paper printout
